We would like to have the Vision, Health, and Dental forms on or before the first day of school. Sports` Physical forms are due before the student can participate in any Sport, this includes practices.

The School Tuition Agreement must be turned in. If you are unable to meet your Financial Guidelines, you must speak to Father Jeff BEFORE registering. (This includes new applications for assistance and those who have received assistance in the past.) The tuition agreement should be signed by Fr. Jeff indicating an adjustment has been made.
