All forms with an asterisk (*) should be returned at Registration COMPLETELY FILLED OUT PRIOR TO COMING. Having these forms filled out ahead of time expedites registration. The return of these forms at Registration will be considered the FORMAL registration of any student attending St. Boniface for the coming school year. If you are unable to register on the designated day, please notify the school office so the names of the students involved will appear on the appropriate class list.

The School Support Agreement MUST BE TURNED IN at REGISTRATION. If you are unable to meet your Financial Guidelines, you must speak to Father Jeff BEFORE registration. Families who have received an “adjustment” in the past, must also speak to Father Jeff and bring in the Guideline Form on registration day indicating the adjustment.
